Is Catalyst 4.3 the latest Windows 98 drivers for that
video card?

If not, download and install the latest driver file from
http://www.ati.com/.

If that isn't the problem, how is the video card displayed
in the Device Manager (accessed by clicking System in the
Control Panel)?

I would start up, press the F8 key just before Windows
loads to bring up the boot menu, boot into Safe mode, go
into the Device Manager and remove the video card and
completely remove all the entries for any device that has
more than one instance showing (except IRQ Holders for PCI
Steering, of which there are supposed to be many).

Windows should then detect and install new hardware, and
you can then install the card's driver file.

Go into Display Properties and select Monitor and make sure it is being
recognized properly.
If it isn't, then variable display refresh rates may not be available.

If the refresh rate was above 60Hz to begin with, you aren't going to see
any difference. Unless you're not of homo sapiens genus of course you may
observe otherwise.
