BLUE SCREEN when USB device plugged in
SOLUTION:
After trying a dozen different things I used google to
serach for UXD USBHUB (the error from the blue screen) and
found a site (USB-DRIVERS.COM) that had a file (usb-win98-
a.zip) which contained four files, one of which was
USBHUB.sys. I loaded only that file in to the
windows/system32 directory replacing the old one and tried
again. Actually I renamed the old one so it would not get
deleted. This time no blue screan. YEAH!!!! Instead I
was prompted to load the drivers for the external drive
that I had plugged into the usb port. I then plugged in a
usb mouse and was again prompted for the drivers which
were already on the system. I guess the drivers I
previously installed from intel were older versions as
well.
Lesson learned: NEVER trust the USB drivers that come with
an old OS like windows 98. First thing to do is look for
the driver that the blue screen is making a refernece to.
This would have saved me many hours of frustration.

Cleaned up the device manager by removing any duplicate

Ran the INTEL USB check utility that indicates the usb in
working properly
Reinstalled the USB drivers using the latest driver
information form Intel,
Removed all usb devices and let the system reinstall them,
Verified I have the most recent version of the bios
Turned on Plug and Play in the bios,
Turned off Power management in the bios
Turned on USB Keyboard in the bios
Can't find USB Legacy support in the bios
Verified that the IRQ Steering is properly installed
Tried to set the Assign IRQ's to auto or system in the
