Fatal Exception 06 when reading CD-R

When I put a CD-R in my CD-RW drive, the Blue Screen of
Death appears with this error message:

A fatal exception 06 has occurred at 0028:00000000. The
current application will be terminated.

After that happens, I hit any key to resume and the whole
system is unstable to the point where I actually have to
kill the power to get it to respond. (fresh boot)

I've searched the Knowledge Base but this particular fatal
exception is not listed.

Can anyone please help?
